Before
your dog(s) may begin training, we need information about both you and your dog(s). This information is very important so please be as accurate as possible. We require that all dogs have their immunizations up to date.
Application Form (#1)
Click the Application Form link below, fill out and then hit the "Submit Paperwork" button.
Legal Agreement Form (#2)
Click on Standard Agreement Form link below, "sign" it, by typing in your email address, checking the approval box, then hit the "Submit" button to send to Pawsitive Power Dog Training.
Veterinarian Clearance Form (#3)
The easiest way is to call your veterinarian and have them FAX your dog's immunization record to (818) 999-9066 . Or you can click on the Veterinarian Clearance Form link below, print it out , have your dog's Vet complete the form, and then mail or FAX it back to Pawsitive Power Dog Training.
